What to Bring
To help us provide the best care possible, please bring the following to your appointment:
- A valid photo ID
- Your dental insurance card (if applicable)
- A list of current medications
- Any recent dental X-rays (if available)
- Completed new patient forms (or arrive early to fill them out)

We also accept a variety of payment options, including cash, check, and major credit or debit cards. If you have any questions about coverage or payment, our team is here to help.

Scheduling & Cancellations
We value your time and strive to stay on schedule. If you need to reschedule or cancel your appointment, we kindly ask that you provide at least 24 hours’ notice so we can accommodate other patients in need of care.
