To save time and make your visit as smooth as possible, we offer the option to complete your patient forms online before your appointment. Filling out your forms in advance allows our team to review your information ahead of time and ensures a quicker, more efficient check-in when you arrive.

The forms available to complete online include the New Patient Form, New Patient FDH (Medicaid), HIPAA Form, and our Financial Policy. Taking a few minutes to fill these out ahead of your visit helps us prepare for your appointment and provide you with a more seamless experience from the moment you walk through our doors.

Been a patient for years, always a great experience. Staff is on it and is very professional. Minimal wait times. All hygienists are always friendly and polite. Dr. Dyer is AWESOME with not only me and my husband, but with my 2 children. The one time I needed to have urgent care for a crown that unglued itself (from a previous dentist), they took me in the same day and glued it right back. Glad we have such a great dentist in our community!
